I would be curious to know what your co-pays and deductibles are for 2004. Our co-pays are $20 for general office visits and $40 for specialists. We have either a $1,000 or $500 deductible depending on the POS1 or POS2 plan. Any feedback would be appreciated. Thanks.

Our co-pays are $20 per office visit, emergency room co-pay is $150 (waived if admitted), 20% for lab/x-rays/hospital stays, 50% Mental Health, 20% Chemical Dependency and for prescriptions $15 (Generic), $25 (Preferred Brand Name), $40 (Non-Preferred Brand Name). Our deductible is $200 per person with an annual $1000 out of pocket maximum.

EE = $229.60 (company pays)
Spouse Only = $297.99 (ee pays)
Family = $497.27 (ee pays, 'cept managers)
Children Only = $220.72 (employee pays)
Dental
$2000 annual cap. $50 deductible & co-pays are 20% Basic & 50% for Major (preventative is covered at 100%).

Family = $77.95 (ee pays, 'cept managers)
Children Only = $43.93 (ee pays)

Our co-pays are $10 per office visit (includes lab if on same visit, another $10 if performed another time), $20 per urgent care, $55 for emergency room, waived if admitted. Precert is required for hospital (24 hours emergency entrance, prior to planned procedure). There's a $250 "penalty" if you don't precertify. For prescriptions $7 (Generic), $15 (Preferred Brand Name), $30 (Non-Preferred Brand Name).
Our employees pay a biweekly premium of:
ee only - $25
ee plus one - $55
ee plus two or three - $80
ee plus four or more - $110
We also have dental and vision benefits included with these premiums that are a bit different, but pretty darn good.
