Unfortunately, I have not been able to unearth any sample SPD's for you. However, you might try asking your plan provider unless you are self-insured. Many plan providers provide that information.

Could I get a copy? I have one for our cafeteria plan, but I don't think it is adequeate. Our premium only plan is "administered" by AFLAC and the SPD they send doesn't cover the group medical insurance.
Also, these supplemental products we offer through payroll deduction (AFLAC) are cancer, Accident insurance, Short term disability. The employers pay NO part of the premium, but just made them available through the pre-tax benefits. Are these subject to COBRA? I've treated them as such. Thanks.
