Thanks ritaanz....I only had scanned copies that my employment attorney sent me. I tried to find the forms on the DOL late last week but had no luck. I love this forum!!!
Thanks for your help. Do you have the link for the designation notice that must be give to the employee? The designation notice informs them that the leave is FMLA or is not FMLA qualified.

That would be the proverbial pink slip.:D Seriously, I don't use a form, just a personalized letter about 2 weeks in advance of the final FMLA date so they aren't caught by surprise. Haven't had any problems so far.
