FMLA - employer designates

Is there a sample form or letter to use to cover all requirements for employers when the employer designates leave as FMLA? I have all of the forms that an employee needs to make the request.

I downloaded a form from the Dept of Labor called "Employer Response to Employee Request for Family or Medical Leave". (OMB No: 1215-0181, Form WH-381) I can't find the site back right now. Search the DOL website or call your local Wage and Hour office and they should be able to help. It's not a pretty form but it serves the purpose, it's free and DOL can't complain about the form since it came directly from them. I'll keep searching for the website so let me know if you can't track it down or if you want me to fax the actual form to you.
