EEO-1 Self ID form

If anyone has a Self-Identification form that they use and would be willing to share, I would love to have a copy. Thanks in advance. Fax# 219-662-4095.

    I sent a short memo out with report forms. Essentially telling the ee that I was required to send this information but they were not required to provide it. However, if they did not provide I would take my best guess as to their gender etc and complete the form. I got all mine back without any problems.

    When new hires are completing "all" the paperwork on their first day, we have them complete a personal information form. Stuff like name, address, phone numbers,who to notify in case of an emergency, etc. We also have the EEOC information on this form and without exception, so far, the ee has completed this section of the form and we use what ever they list.
