Background check consent form

The owner of the company I work for has requested that I perform background checks on all new employees. Is there a standard form out there for North Carolina that I can get my candidates to sign prior to my doing the check?

We have one with suggested verbiage under the Fair Credit Reporting Act that we have the candidate sign. If you send me your e-mail I can forward it on to you. It's in Word format.
