Does anyone offer AFLAC to your employees? If so, do you complete a Schedule A to go with the 5500 for AFLAC?
Thanks

The insurance agent/broker that introduced your company to AFLAC should provide you with the 5500 and answer any of your questions. Give them a call. It's is part of the package when your company agreed to have them on board.

AFLAC actually prepares the 5500 for us. No charge.

Ditto here! They even do my SRA's (salary redirection agreements) since we deduct premiums out of pre-tax dollars.
