New COBRA notification and election letters

I've posted this under benefits but received no responses so I'm trying here. Does anyone have these letters in Microsoft Word format? If so I'd appreciate receiving them via email to [email]jlutz@lsnj.org[/email]. I've tried copying them from the internet but haven't been successful. Thanks so much.

C'mon guys; surely some of you have these documents and can forward them or link them here somehow. If not, can someone respond with the jest of your forms or suggest a website? Ours are handled corporately.

I sincerely thank those of you that sent me the documents you are currently using for COBRA notification and election however, none of those appear to be the newly designated forms that the DOL is suggesting we begin to use now, (even before the new regs are passed). If anyone does have those in Microsoft Word format please do send them along. Thank you so much.

I suppose it's OK to begin using the new notices, but they don't go into effect until Jan.1, 2004 and could still change. Just make sure you keep up on the changes so you can then download the revised revised version or maybe even the revised revised revised version so you can maintain your "safe harbor" status. I think the DOL needs something to do.

In fact, the DOL has "recommended" that employers begin using the new letters immediately.

Many thanks to all of those who answered my plea. I now have the new documents in word and am adapting the language to fit our company! :-)
