Applicant flow data

Our bank needs to compose an applicant flow data form to attach to our applications. EEO requires that we track applicants but does not have a specific form for us to use. We want to make sure we are legal and correct with our wording. I would appreciate your help. Thanks

I have a form that attaches to the application, requesting EEO information. If this is what you need, let me know and I'll email it to you tomorrow - it's on the computer at my other plant.
