We are a small company. The employees get ten days at the first of each year of their anniversary. New hires have to wait 90 days, then they get the ten days. Illnesses of three or more days may require a doctor's statement.
Supplying a doctors note has a positive and negative side. Many times a simple illness such as a bad cold or a headacke does not result in a doctors visit. If you require a doctors note that means that the employee has to pay $ to satisfy your requirements. The requirement makes more sense for those employees who are on longer term illnesses or who are abusing your policy. Sometimes it is best to be flexible with the notes and require them when necessary - to do otherwise comes across as the employer distrusting everyone when only a minority of employees are a problem.
